Common deficiencies in a clinic environment

It may seem unusual to reference vials twice in the same newsletter, however this topic is extremely important. Violations can negatively impact the health of your patients as well as that of your business!

In the last several months, we have cited vial deficiencies numerous times. Any person withdrawing contents from an unopened vial should first check the expiration date, then check to see if the vial is single does or multidose. If the vial is a single dose vial, then immediately discard it once you have withdrawn the contents. That way there is no chance it will be used again. This is critically important. A dated single dose vial is a condition level deficiency and will cause you to fail your survey. Remember there is no preservative in that vial. It is meant for one patient and one patient only.

Use this opportunity to retrain your staff. The CDC’s One & Only Campaign is a public health effort to eliminate unsafe medical injections. Through targeted education and awareness efforts, the One & Only Campaign empowers patients and healthcare providers to insist on nothing less than safe injections – every time, for every patient. Let’s keep our patients safe!
