A medication record is maintained for all patients and documentation can be found in the patient medical record.

Remedy
Patients/caregivers are asked to provide medication profiles, including current medication and over-the-counter drugs or supplements, at the initial pharmacy encounter. These medication profiles are maintained and documented for all patients. Appropriate staff is trained to collect and document this information and are knowledgeable of the process.

Having complete and current medication profile documentation for patients ensures, for example, that contraindications with newly prescribed medications or potential adverse reactions are more easily identified, and patient care is provided at the highest possible level.
